docs: document Format 4 and Format 5 todo detection patterns

Add native checkbox (☐/◐/☒) and Claude Code checkmark-based
(✔ Task #N) formats to CLAUDE.md and ralph-wiggum-guide.md,
matching the recently added detection code.
