fix: mobile toolbar button sizing and accessory bar commands

- Fix Run Claude button being squeezed on mobile (use fit-content width)
- Remove Send button from accessory bar
- Add /init, /clear, /compact buttons with confirmation dialogs
- Center toolbar buttons instead of stretching
