fix: remove New Session panel entirely

- Remove New Session slide-up panel from HTML
- Remove createNewSession, hideNewSessionPanel, createSessionFromPanel functions
- Remove Ctrl+N keyboard shortcut
- Remove "New Session" from help modal
- Clean up loadQuickStartCases and closeAllPanels references
