- README.md: Update API endpoints to /ralph-state and /ralph-config
- ralph-wiggum-guide.md: Update class names (RalphTracker, RalphLoopState),
  file paths (ralph-tracker.ts), SSE events (session:ralphLoopUpdate), and
  API endpoints throughout
